U.S. Benefits Summary for Separating Employees

Coverage will remain in effect through the last day of the month in which you terminate employment with the Company. You may continue this coverage under COBRA. You will receive a COBRA information packet from our COBRA Administrator (HealthEquity) mailed to your home address within approximately 2 weeks from your last day of work.

HSA contributions will cease upon termination of employment. All funds remain in your account. Funds will carry over from year to year. You may continue to use the funds for qualified expenses.

Anthem Members: Your account will be transferred to WealthCare’s portal at  www.wealthcare.com. WealthCare will mail you the login instructions. There will be a monthly fee of $2.50 deducted from your account balance as long as the account remains active unless you are enrolled in the Anthem HDHP under COBRA.

Kaiser Members: Your account will be transferred to kp.org/healthexpense. There will be a monthly fee of $3.25 deducted from your account balance as long as the account is active unless your average account balance is $2,000 or higher.

Health Care FSA and Limited Purpose FSA contributions will end upon termination of employment. You may request reimbursement for qualified expenses incurred prior to your termination date. You have 90 days from your termination date to file claims.

You may request reimbursement for qualified expenses related to fertility treatment incurred prior to your termination date. You have 90 days from your termination date to submit for reimbursement.

Coverage generally ends on the last day of the month in which you terminate employment with the Company.

Coverage will end upon termination of employment. You may convert this coverage to an individual insurance policy. Complete “Mutual of Omaha Life Conversion Form” for Basic Life & AD&D insurance conversion and “Mutual of Omaha Term Life Portability Request Form” for Supplemental Life and AD&D insurance and submit the completed form(s) to Mutual of Omaha. Refer to the form(s) for detailed instructions.

Coverage will end upon termination of employment with the Company. Complete “Mutual of Omaha Disability Continuation Request Form” and submit the completed form to Mutual of Omaha if you need continued disability insurance protections. Refer to the form for detailed instructions.

You will lose access to the funds upon termination of employment with the Company. You will not be able to transfer the balance or access the funds beyond that date.

Refer to the "MetLife Legal Plans Portability Notice" and "Aura Identity & Fraud Protection Portability Notice" for detailed instructions on how to continue your coverage after your termination date.

Complete “Mutual of Omaha Accident Continuation Request Form” for Accident insurance continuation, “Mutual of Omaha Critical Illness Continuation Request Form” for Critical Illness insurance continuation, and "Mutual of Omaha Hospital Indemnity Continuation Request Form” for Hospital Indemnity insurance continuation. Once complete, please submit the completed form(s) to Mutual of Omaha. Refer to the form(s) for detailed instructions.

If you have more than $7,000 in your account, you may leave your money in the Company’s 401(k) plan or you may take a distribution including rollover to an IRA or another 401(k) account. If you have less than $7,000 in your account, you must take a distribution or rollover. To initiate a distribution or rollover, visit the NWPS portal at www.yourplanaccess.net/nwps or call NWPS Customer Service at (888) 700-0808. If you have an outstanding loan from the 401(k) plan, the outstanding balance will be treated as a taxable distribution if repayment is not made within 90 days from your termination date. Call NWPS Customer Service at (888) 700-0808 for repayment instructions. 

If your home address or personal email changes, please notify the Human Resources Department at na-hr-operations@santen.com. This will ensure the Company has your accurate contact information including home address to send you your W-2 form and other tax-related material after the end of the calendar year.
